2022/2023 Season Feedback Form
We are asking for feedback from our membership to offer insight into our current Coaches, to help make future decisions.
Feedback is important as it helps us as an organization gauge what is working and what we can improve on.
This feedback will be kept strictly confidential and anonymous. coaching staff will not be provided direct quotes from any of the evaluations, comments will be grouped into themes and the ratings will be tabulated for an average rating.
This feedback form will close on February 5, 2023.
